                      વિદ્યાર્થીઓના રસના ક્ષેત્રો , વ્યક્તિગત તેમજ સામાજિક ગુણો,મૂલ્યો , શારીરિક કૌશલ્ય વગેરે  મૂલ્યાંકન માટે વ્યક્તિત્વ વિકાસ પત્રક "B" શિક્ષકે ભરવાનું હોય છે. આ પત્રક દ્વારા વિદ્યાર્થીઓના વ્યક્તિત્વના વિવિધ પાસાઓ જેવા કે શારીરિક, ,માનસિક સામાજિક ,ભાવાત્મક ઉપરાંત  કૌશલ્ય અને મૂલ્યો જેવા પાસાઓનો  સમાવેશ કરવામાં આવેલ છે. આ પત્રકમાં કુલ 4 ક્ષેત્રમાં કુલ 40  વિધાનોને આધારે ધોરણ 3 થી 5 માં સત્રવાર  દરેક વિધાન માટે 5 ગુણ લેખે  તથા ધોરણ 6 થી 8 માં દરેક વિધાન માટે 10 ગુણ લેખે શિક્ષકે વિદ્યાર્થીઓનું મૂલ્યાંકન કરવાનું રહે છે. આ પત્રકમાં દર્શાવેલ 40 વિધાનો પૈકી 9  છોડેલ છે. શિક્ષકે પોતાની  શાળામાં આંતરે રજુ કરેલ 31 પ્રવૃતિઓ અગત્યની કોઈ પ્રવૃત્તિ કરાવતા હોય તો તેવી પ્રવૃત્તિઓનો ઉલ્લેખ આ 9 ખાલી વિધાનોમાં કરી તેનું મૂલ્યાંકન કરી શકશે.

➽ SCE PATRAK "A" 
  • ધોરણ  3 થી 8 માં ધોરણવાર,વિષયવાર અને સત્રવાર અલગ-અલગ પત્રકો ભરવાના છે.
  • જે વિષયમાં 20 કરતા વધારે અધ્યયન ઉપલબ્ધીઓ હોય ત્યાં પ્રતિનિધિરૂપ પસંદ થયેલ 20 અધ્યયન ઉપલબ્ધીઓના માત્ર ક્રમાંકો પત્રકમાં દર્શાવવા.
SCE PATRAK 'A' RACHANATMAK MULYANKAN PATRAK FOR STD 3 TO 8@gcert.gujarat.govt.in

  • જે વિષય માં 20 કરતા ઓછી અધ્યયન ઉપલબ્ધીઓનું હોય ત્યાં તમામ ઉપલબ્ધીઓનો ક્રમ પત્રકમાં દર્શાવવો.
  • જે તે અધ્યયન ઉપલબ્ધીઓનું મૂલ્યાંકન કરતા અધ્યયન ઉપલબ્ધીઓ સિદ્ધ થઇ હોય તો ખરા (✓) ની નિશાની કરવી.થોડી ઘણી કચાશ રહી હોય તો પ્રશ્નાર્થ (?)  ની નિશાની કરવી. જો વિદ્યાર્થીને કશું જ ના આવડે એટલે કે અધ્યયન ઉપલબ્ધી સિદ્ધ ના થાય તો ચોકડી (X) ની નિશાની કરવી. 
  • જે તે વિષયમાં જેમ જેમ અભ્યાસક્રમ આગળ વધે તેમ તેમ જે તે અધ્યયન ઉપલબ્ધીઓ/પ્રકરણનું શિક્ષકે વિવેકબુદ્ધિથી  બાળકોના અવલોકનના આધારે મૂલ્યાંકન કરવું.
  • જે વિદ્યાર્થીને ચોકડી ની કે પ્રશ્નાર્થ ની નિશાની થઇ હોય તેના માટે તરત જ અન્ય પદ્ધતિ કે પ્રયુક્તિનો ઉપયોગ કરી પુનઃઅધ્યાપન કરી પુનઃમુલ્યાંક્ન કરવું.તેમાં વિદ્યાર્થી ઉચ્ચ સિદ્ધિ મેળવે ત્યારે તેના આધારે સબંધિત નિશાની કરવી.
  • ખરાની નિશાની અધ્યયન ઉપલબ્ધીઓની ઉત્કૃષ્ટ કક્ષાની સિદ્ધિ દર્શાવે છે. પ્રશ્નાર્થની નિશાની દર્શાવે છે કે બાળક અધ્યયન ઉપલબ્ધીઓને સમજવા માટે પ્રયત્નશીલ છે. જ્યારે ચોકડી(X )ની નિશાની દર્શાવે છે કે બાળક અધ્યયન ઉપલબ્ધિઓની સમજ અભિવ્યક્ત કરી  શકતું નથી. 

  • એક જ વિદ્યાર્થી માટે જે તે અધ્યયન ઉપલબ્ધી ખાનામાં એકથી વધુ ચોકડીની નિશાની ,એક થી વધુ પ્રશ્નાર્થની નિશાની હોય શકે ,પરંતુ ખરાની  નિશાની માત્ર એક જ વાર આવશે.ખરાની  નિશાની આવે તે માટે શિક્ષકે સંનિષ્ટ રેતે શક્ય પ્રમાણમાં પ્રયત્ન કરવા.
  • વિદ્યાર્થીના નામ સામે સત્રાન્તરે દરેક નિશાનીઓની કુલ સંખ્યાની ગણતરી કરી,'X ', '?', '✓' ની નિશાનીની સંખ્યા જે તે ખાનામાં દર્શાવવી. 
  • પ્રથમ સત્રમાં પસંદ કરેલ અધ્યયન ઉપલબ્ધીઓ સત્રમાં પણ આવી શકે.
  • નીચે આપેલ સૂત્રની મદદથી 40 માંથી ગુણ મળશે. આ ગુણપત્રક - A ના 40 માંથી મેળવેલ ગુણના કોલમમાં દર્શાવવા

                                                સત્રની કુલ ક્ષમતાઓની સંખ્યા  

                   દા.ત.     કોઈ વિદ્યાર્થીએ ધોરણ 5 ના ગણિતના વિષયમાં 16 અધ્યયન ઉપલબ્ધીઓમાંથી 12 અધ્યયન ઉપલબ્ધીઓની અંદર '✓' ની નિશાની મેળવી છે. તો તેની ગણતરી નીચે પ્રમાણે થાય.

  • એક જ વિદ્યાર્થી માટે જે તે અધ્યયન ઉપલબ્ધીઓના સામેના ખાનામાં દર્શાવેલી કુલ નિશાનીઓ શિક્ષકે તે વિદ્યાર્થી માટે તે અધ્યયન ઉપલબ્ધીઓ સિદ્ધ કરવા માટે કરેલા પ્રયત્નોની સંખ્યા દર્શાવે છે.
